A cross-sectional study to ascertain what the Singapore population would regard as material risk in the anaesthesia consent-taking process and identify demographic factors that predict patient preferences in medical decision-making to tailor a more patient-centered informed consent. A survey was performed involving patients 21 years old and above who attended the pre-operative evaluation clinic over a 1-month period in Singapore General Hospital. Questionnaires were administered to assess patients’ perception of material risks, by trained interviewers. Patients’ demographics were obtained. Mann–Whitney U (...) test and Kruskal-Wallis one-way analysis of variance was used. Statistical significance was taken at p < 0.05. Four hundred fourteen patients were eligible of which 26 refused to participate and 24 were excluded due to language barrier. 364 patients were recruited. A higher level of education, being employed and younger age group are factors identified in patients who wanted greater participation in medical decisions. Gender, marital status, type of surgery, and previous surgical history did not affect their level of participation. The complications most patients knew about were Nausea, Drowsiness and Surgical Wound Pain. Patients ranked Heart Attack, Death and Stroke as the most significant risks that they wanted to be informed about in greater detail. Most patients wanted to make a joint decision with the anaesthetist, instead of letting the doctor decide or deciding for themselves. Discussion with the anaesthetist is the preferred medium of communication compared to reading a pamphlet or watching a video. Age and educational level can influence medical decision-making. Despite the digital age, most patients still prefer a clinic consult instead of audio-visual multimedia for pre-operative anaesthetic counselling. The local population appears to place greater importance on rare but serious complications compared to common complications. This illustrates the need to contextualize information provided during informed consent to strengthen the doctor-patient relationship. (shrink)

Recent evidence shows that dynamic instability is the dominant mechanism for the assembly of pure tubulin in vitro and for the great majority of microtubules in the mitotic spindle and the interphase cytoplasmic microtubule complex. The basic concepts of this model provide a framework for future characterization of the molecular basis of spatial and temporal regulation of microtubule dynamics in the cell and the function of microtubule dynamics in motile processes such as chromosome movement.

The complex mechanisms by which nonlinear classical conservative systems undergo a transition from quasiperiodic to chaotic behavior are now fairly well understood. This transition is associated with a breakdown of quasi-constants of motion (KAM surfaces). There is growing evidence that similar mechanisms may govern the behavior of quantum systems. While K-type mixing behavior has not yet been found, there does appear to be a transition associated with the destruction of a quantum quasi-constant of motion (quantum KAM states) which changes qualitatively (...) the spectrum of quantum systems. This work is of the view that the question of being is not only a problem in Western philosophy but also in African philosophy. It, therefore, posits that being is that which is and has both abstract and concrete aspect. The work arrives at this conclusion by critically analyzing and evaluating the views of some key African philosophers with respect to being. With this, it discovers that the way that these African philosophers have postulated the idea of being is in (...) the same manner like their Western philosophers whom they tried to criticize. This work tries to synthesize the notions of beings of these African philosophers in order to reach at a better understanding of being. This notion of being leans heavily on Asouzu’s ibuanyidanda ontology which does not bifurcate or polarize being, but harmonizes entities or realities that seem to be contrary or opposing in being. The overall objective of this research is to determine the religious perceptions and its levels of university youth and to interpret the findings from the point of view of their psychological features. This research consists of four chapter. These are: introduction, first chapter consist of discussions of the basic concepts such as religiosity, spirituality, second chapter that the data obtained from the survey was assessed the relationships with hypotheses and conclusions in which the findings were interpreted. The study’s population consists (...) of the students at the undergraduate level in faculties of the Erciyes University in 2011-2012. A questionnaire was administered on 553 students, chosen randomly in this population. The survey contains descriptive and Likert style items. As independent variables in this study, branches of science, faculty, department, class, age, gender, living environment for the longest time before coming to university, graduated high school, mother's and father's education level and the accommodation place during training have been taken. When analyzing the data, percentages, frequency distributions, descriptive statistics, analysis of variance (one-way ANOVA) and T-Tests and Chi-Square (x²) techniques were used. As a result of this research hypotheses associated with gender, age, branches of science, faculty and department were confirmed on the other hand the others were rejected. (shrink)

This is a general account of the Cappadocian Christian Father Gregory of Nyssa (c. 335 - c. 395 CE) as a philosopher. The article is divided into a discussion of his life and his views on God, the world, humanity, history, knowledge, and virtue. A common thread, which would later be systematized in the Palamite essence-energies distinction, is traced in all these topics. Of particular interest to philosophers are comparisons with John Locke and Immanuel Kant.

The development of genomic technologies has seemed almost magical. Excitement about it, both in medicine and among the public, stems from the belief that genomic techniques will illuminate the causes of health and disease, will lead to effective interventions for both rare and common genetic conditions, and will inform reproductive decision‐making. Novel diagnostic tools, however, are often deployed before targeted therapies are developed, tested, or available and before their psychosocial implications are explored. Newer technologies such as prenatal whole exome screening (...) are seen as offering “decisional autonomy” to expectant parents, although such technologies identify information about genetic sequencing that may not have clear meaning. The “therapeutic gap” between the ability to conduct genetic sequencing and the ability to fully understand what the test results mean, much less what treatments to offer, leaves families with complex and unclear information they cannot act upon with confidence during pregnancy. In this essay, we will consider the psychosocial and ethical implications of such assumptions—and of the uncertain information produced by these technologies—for individuals and families and for societal aspects such as medical service usage and demographic inequities. Although children’s negative affectivity is a temperamental characteristic that is biologically based, it is framed within and shaped by their emotional environments which are partly created by maternal emotion expressivity in the family. Children, in turn, play a role in shaping their family emotional context, which could lead to changes in mothers’ emotion expressivity in the family. However, these theorized longitudinal bidirectional relations between child negative affectivity and maternal positive and negative expressivity have not been studied from toddlerhood to early (...) school-age. The current study utilized a cross-lagged panel model to examine the reciprocal relations between children’s negative affectivity and maternal expressivity within the family over the course of early childhood. Participants were 140 mother–child dyads. Mothers reported the positive and negative expressivity in the family and children’s negative affectivity in toddlerhood, preschool, and school-age. Maternal negative expressivity and child negative affectivity at T1 were significantly correlated. Maternal negative expressivity at T1 significantly predicted child negative affectivity at T3. Children’s negative affectivity at T2 significantly predicted mothers’ negative expressivity at T3. Mothers’ positive expressivity was not related to children’s negative affectivity at any of the three time points. The findings demonstrate the reciprocal relations between children’s negative affectivity and maternal negative expressivity in the family, suggesting the importance of the interplay between child temperament and maternal expressivity within the family emotional context. The Ontology for Biomedical Investigations (OBI) is an ontology that provides terms with precisely defined meanings to describe all aspects of how investigations in the biological and medical domains are conducted. OBI re-uses ontologies that provide a representation of biomedical knowledge from the Open Biological and Biomedical Ontologies (OBO) project and adds the ability to describe how this knowledge was derived. We here describe the state of OBI and several applications that are using it, such as adding semantic expressivity to (...) existing databases, building data entry forms, and enabling interoperability between knowledge resources. OBI covers all phases of the investigation process, such as planning, execution and reporting. It represents information and material entities that participate in these processes, as well as roles and functions. Prior to OBI, it was not possible to use a single internally consistent resource that could be applied to multiple types of experiments for these applications. OBI has made this possible by creating terms for entities involved in biological and medical investigations and by importing parts of other biomedical ontologies such as GO, Chemical Entities of Biological Interest (ChEBI) and Phenotype Attribute and Trait Ontology (PATO) without altering their meaning. OBI is being used in a wide range of projects covering genomics, multi-omics, immunology, and catalogs of services. OBI has also spawned other ontologies (Information Artifact Ontology) and methods for importing parts of ontologies (Minimum information to reference an external ontology term (MIREOT)). The OBI project is an open cross-disciplinary collaborative effort, encompassing multiple research communities from around the globe. To date, OBI has created 2366 classes and 40 relations along with textual and formal definitions. The OBI Consortium maintains a web resource providing details on the people, policies, and issues being addressed in association with OBI. (shrink)
